# Assessments, Accomplishments & Lessons

Record partnership assessments, accomplishments, and lessons learned on a partner report in PEARS.

Both of these sections begin with a Yes/No question. Answering it is required before the report can be marked complete, and answering **Yes** requires at least one entry.

**TIP:** Answering **No** marks the section complete with nothing recorded. That is a deliberate answer — "we did not assess this partnership this period" — rather than an empty section.

## Assessments

The **Assessments** section records formal evaluation of the partnership.

#### Answer the prerequisite question

**Do you have assessments to report?** Select **Yes** or **No**. The assessments table appears only when you answer **Yes**.

#### Add an assessment

Click **Add** to open the assessment form.

#### Select the Assessment Tool Used **\***

In the **Assessment tool used** dropdown, select the tool used to report on this partnership. If you select an **Other** option, a **Please specify other assessment tool** field appears and is required.

#### Set the Date

In the **Date** field, select when the assessment was carried out.

#### Describe the Results

In the **Brief Description of Results** field, record the assessment's outcome — a score, a level, or a short summary.

#### Save

Click **Save**. The assessment appears in the table. Add as many as you need, one at a time.

**NOTE:** The assessment tool list offers partnership-type tools only, and it has no blank or **None** option — whether an assessment happened at all is what the section's Yes/No question answers.

## Accomplishments and Lessons

The **Accomplishments & Lessons** section records what the partnership achieved and what you learned from it. Both live in one table, distinguished by the entry's type.

#### Add an entry

Click **Add** to open the form.

#### Select the Type **\***

Choose whether this entry is an accomplishment or a lesson learned.

#### Enter a Description **\***

Describe the accomplishment or lesson learned.

#### Set the Date (optional)

Select when it occurred.

#### Save

Click **Save**. Add each accomplishment and lesson one at a time.
