# Edit an Action Plan Report

Follow the steps below to edit an existing action plan report.

#### Navigate to Action Plan Reports

From the PEARS homepage, hover over the **Reflect** menu and click **Action Plan Reports**.

#### Select an Action Plan Report

The Action Plan Reports list displays available records. Click the report you want to edit.

#### Click Edit

On the action plan report detail page, click the **Edit** button.

#### Edit the Action Plan Report

The General Information page loads. Modify the report details as needed across sections including General Information, Custom Data, Situation Updates, and Outcome Evidence.

#### Save

Click **Save and Continue** to save changes and proceed to the next section, or click **Save** to save and return to the record detail page.

**NOTE:** The assigned reporting period and selected Action Plan cannot be changed after the record is created.

**NOTE:** You may only edit Action Plan Reports you created in the current reporting period, unless you are an editable user or Data Manager. Contact your Data Manager to access records from previous reporting periods.
