# Sustainability

Document sustainability planning for a PSE Site Activity, including sustainability mechanisms, policy levels, stakeholder groups, and assessment results.

The Sustainability section captures whether any efforts have been taken to sustain the PSE changes over time and documents the mechanisms, policy levels, stakeholder groups, and assessment results that support long-term maintenance of the effort at this site or organization.

#### Indicate if sustainability efforts have been taken

Answer **Have any efforts been taken to support sustainability of the PSE changes over time?** **\*** by selecting **Yes** or **No**.

**NOTE:** If you select **No**, the remaining sustainability fields are hidden and any sustainability details already recorded are removed when you save. You may proceed to the next section.

#### Rate sustainability mechanisms

Under **Sustainability Mechanisms**, answer **Which of the following are in place to ensure the sustainability of this PSE work in future years?** **\*** by selecting **No Plans to Adopt**, **Plan to Adopt**, **In Process / Partially Adopted**, or **Fully in Place** for each mechanism:

* **An organization or group that is not dependent on SNAP-Ed funding has assumed responsibility for sustaining the efforts**
* **A dependable, on-going source of funding and/or support (other than SNAP-Ed) has been identified**
* **One or more policies were adopted, requiring the changes to be maintained**
* **A monitoring and reporting system has been implemented**
* **Support from stakeholders**
* **Other Mechanism** — Optionally, name an additional mechanism in the box at the start of this row and rate it. A named mechanism must also be rated, and a rated mechanism must also be named.

Extension organizations see the first two mechanisms worded with **your organization** in place of **SNAP-Ed**.

#### Select policy levels

Under **Select the level(s) at which policies require the PSE changes be maintained**, check any of **Site**, **Agency/Organization**, **Local Government**, **State Government**, **Federal Government**, or **Tribal Government**.

#### Select stakeholder groups

Under **Select all stakeholder groups whose support will help to sustain this effort**, check any of **Consumers/students/residents**, **Administrators/owners/managers**, **Committees/school boards**, **Staff/teachers/employees/farmers**, **The public/local community**, **Elected official**, **Private community-based organizations**, or **Other (please specify)**. Checking **Other (please specify)** reveals a text box **\*** where you must name the group.

#### Select the framework or assessment tool used

Select the **Sustainability framework or assessment tool used for this reporting year**. Choosing **Other, please specify** reveals an **Other tool** **\*** field for the name of the tool. Select **None** if no framework or assessment tool was used.

Under **Please provide a brief description of your assessment results**, summarize what the framework or assessment tool showed about the sustainability of this PSE work.
