Registration Form Templates and Blocks

Manage reusable event registration form templates and blocks in PEARS.

Registration Form Templates and Blocks are reusable form components that can be applied when creating event registration types. This tool allows event administrators to create, view, copy, and manage form templates and blocks.

Permission required: Event Administrator. Organization Administrators do not have access to this page by default — the Event Administrator role must be assigned.

Getting Started

To access the templates, click the cog icon in the top navigation bar, select Organization Settings, then choose System Lists & Templates → Event Form Templates. The list view displays all templates and blocks for your organization.

Templates vs. Blocks

PEARS uses two types of reusable form components:

  • Templates — Complete registration forms that can be used as the starting point for a new event registration type. A template contains all the fields needed for a full registration form.

  • Blocks — Reusable form sections that can be added to an existing form. A block contains a subset of fields that can be inserted into any registration form.

Filters

Use the filter options to narrow the list:

  • Templates — Show only complete form templates

  • Blocks — Show only reusable form blocks

Create a Template or Block

To create a new form template or block from scratch:

Click Add Template

From the Manage Event Form Templates list view, click the Add Template button.

Enter template details

Enter a Name for the template and select whether it is a Template (complete form) or Block (reusable section).

Build the form

The form builder opens with a drag-and-drop interface. Use the following tools to design your form:

  • Add fields — Drag field types from the left panel onto the form canvas. Available field types include text inputs, dropdowns, checkboxes, radio buttons, date pickers, and file uploads.

  • Configure field properties — Click a field to set its label, placeholder text, help text, and whether it is required.

  • Set validation rules — Define input validation such as character limits, number ranges, or required formats.

  • Add conditional logic — Configure fields to show or hide based on responses to other fields. This allows you to create dynamic forms that adapt to the registrant's answers.

  • Reorder fields — Drag fields up or down to change their order on the form.

  • Remove fields — Click the delete icon on any field to remove it from the form.

Save the template

Click Save to save the template. It will now appear in the list and can be applied when creating event registration types.

Copy a Template or Block

To create a copy of an existing template or block:

Select the Template

From the list view, click the Copy action on the template or block you want to duplicate.

Enter a Name

Enter a Name for the copy. The name is pre-filled with the original name followed by "(Copy)".

Select the Type

Choose whether the copy should be a Template or a Block. You can convert a template to a block (or vice versa) during the copy.

Save

Click Save to create the copy. You will be redirected to the form builder to edit the copied template or block.

TIP: Copying a template duplicates all of its form fields and options. You can then modify the copy without affecting the original.
